Insurance & payment. KeyTone Medical is out of network with all insurance plans, including Medicare. Clinical visits and procedures are self-pay, with the cost discussed before anything is scheduled; on request we provide a superbill — an itemized, coded receipt — for you to submit to your plan for reimbursement under its out-of-network guidelines. Consultations begin at $200 and include review of your existing imaging (please send all imaging reports before your appointment), and procedure pricing is provided before anything is scheduled. A $200 deposit holds your appointment; it is applied toward the cost of your care and is forfeited in the event of a no-show or late cancellation. HSA and FSA cards are accepted, and financing is available through CareCredit. This applies to clinical care only. QME and medico-legal evaluations are billed through the workers' compensation process.
